NHP SelectChoice
A new choice for people with Medicare, NHP SelectChoice is the right choice for those who want comprehensive medical coverage with a $0 monthly plan premium. The plan provides broad coverage, no or low co-payments, and a $2,500 maximum on out-of-pocket expenses for Medicare-covered services.
As an NHP SelectChoice member, you’ll have the freedom to go to any doctor, specialist, or hospital that accepts Medicare and accepts the terms, conditions and payment rate of the plan. There is no monthly plan premium. You just continue to pay your Medicare Part A, if applicable, and Part B premiums.

Travel benefits
With Network NHP SelectChoice, you’ll receive the right care at the right time from the right provider no matter where you are. The plan has no network requirement, so you can travel in comfort knowing that your health care is there when you need it. If you’re traveling outside the state of Wisconsin and need medical care, please show your NHP SelectChoice member ID card and you’ll receive the same level of benefits as you would at home from any doctor, specialist, or hospital that accepts Medicare and accepts the terms, conditions and payment rate of the plan.

Inpatient hospitalization
You shouldn’t have to suffer a financial hardship — or have to limit your choices —when a serious illness results in a hospital stay. With NHP SelectChoice, you’ll enjoy an affordable, fixed copayment of $150 a day (days 1 – 4) with an annual out-of-pocket maximum of just $1,700 for inpatient treatment if you are hospitalized.

Diabetes self-monitoring training and medical nutrition therapy You pay no co-payment for Medicare-covered Diabetes self-monitoring training and nutrition therapy. And, as an NHP SelectChoice member, you have the freedom to choose any provider you want — there is no network requirement — that accepts Medicare and accepts the terms, conditions and payment rate of the plan.

Diagnostic tests, X-rays and lab services
As a NHP SelectChoice member, you pay:
- $0 for Medicare-covered:
- Lab services
- Diagnostic procedures and tests
- 10% of the cost for Medicare-covered X-rays
- 10% of the cost for Medicare-covered diagnostic radiology services
- 20% of the cost for Medicare-covered therapeutic radiology services

Emergency and urgently needed care NHP SelectChoice covers you for Emergency and Urgently Needed Care anywhere in the world. You never pay more than $50 for each Medicare-covered emergency room visit. If you are admitted to that hospital for the same condition within one day of the Emergency room visit you do NOT pay the $50 for the emergency room visit.

Health and wellness education – after-hours information NHP SelectChoice offers a variety of health education and disease management services, including newsletters, smoking cessation programs, and a Nursing Hotline. Enroll NOW
As long as you have Medicare Part A and Medicare Part B, you can enroll in the NHP SelectChoice plan online right now by clicking here. Once your enrollment form has been approved, your coverage will be effective January 1, 2008 for those who enroll before December 31, 2007. After that, enrollees' coverage will be effective the first of the month following their enrollment submission.
Your premiums
When you join NHP SelectChoice, your monthly plan premium will be $0. Members must continue to pay Medicare Part B premiums.
Like a Medicare supplement, NHP SelectChoice does not use a provider network. However, a Medicare Advantage Private Fee-For-Service plan works differently than a Medicare supplement plan. Your doctor or hospital must agree to accept the plan’s terms and conditions prior to providing health care services to you, with the exception of emergencies. If your doctor or hospital does not agree to accept our payment terms and conditions, they may not provide health care services to you, except in emergencies.
